Artist Market Application Check List



 



Before you send your Artist Market Application for the 5th Annual Grant Park Summer Shade Festival, please review the check list below.  When all items are in place, your application is ready to submit.  Sorry, incomplete applications will not be forwarded to the Selection Committee or be considered as “submitted”.  Deadlines apply to complete applications.



 



3 Hard copy photos or slides or digital images (CD) (minimum) Representative of work to be exhibited at Festival;





1 Hard copy photo or slide or digital image (CD) (minimum) of exhibit space/presentation;







Signed and dated Artist Hold Harmless Agreement;



Copy of Manufacturers  Flammability Statement for Your 10x10 tent;



 Self addressed and stamped envelope;



 



Business or personal check for booth rental fees;  check payable: Grant Park Summer Shade Festival;



 



Non Refundable Business or personal check for Artist Jury Fee; check payable: Grant Park Summer Shade Festival; 



 



Completed application.  All information requested on application is required.  Failure to include all information  with accompanying documents, photos and payments will be considered an incomplete application. 



 



Checks returned due to Insufficient Funds will void application 



We can accept Master Card, ViSA or American Express for payment of entry/jury fees.  A $5.00  service charge is added.
